Notes in filling in the membership form

1. In filling out the form, please try to be as brief as possible.
2. Items with boxes
   
   You may choose to keep private information in certain fields undisclosed by checking the box. (Please note that the items marked with an asterisk are for office use only.)
3. Title
   Examples would include: “professor emeritus”, “professor”, “associate professor”, “assistant professor”, “assistant”, “instructor”, “graduate student”.
   If you are a graduate student, please simply say that you are a graduate student, without any further details such as a master’s student or doctoral student.
